About Family Formation

Family Formation, also known as Adoption and Assisted Reproduction Law Offices of Diane Michelsen, has been instrumental in the creation of more than 3,000 families through adoption, surrogacy and ovum donation. If you are between 21 and 34 years of age, and have had a previous childbirth, we invite you to apply for our Gestational Surrogacy program. The women who have applied for and have been accepted into our program want to give the gift of life to a couple who cannot bear children on their own. These women have found it emotionally fulfilling to bear a child for another family as a Gestational Carrier (also referred to as a "Surrogate"). They also found that, in addition to feeling good about what they did, they were later able to make many wonderful things possible for themselves and for their families.

In a Gestational Surrogacy, the Gestational Carrier bears a child created by In Vitro Fertilization (IVF), from the egg of the Intended Mother (or Egg Donor) and the sperm from the Intended Father (or Sperm Donor). To qualify, we will ask you to complete a medical and psychological screening process. During this process, we will be considering your medical and pregnancy history, family genetics, and height/weight proportions. If you are selected to be a Gestational Carrier, the compensation is $23,000 (plus additional amounts for gestating multiples, delivery via c-section, certain expense reimbursements, etc.).
